Purpose
To engaging diverse people, in order to reduce their cardiovascular disease (CVD) risk and to manage their pre-existing conditions.

Activity
We firstly did desk research on existing evidence, guidance, best practice and examples.
To gain insights into what is most likely to resonate and work, we then held conversations through meetings and workshops with people from the target populations in Greater Manchester.
We then collated and synthesised the principles, ideas, guidance and examples on how to effectively reach out to and to positively engage people.
This was then turned into a section for a toolkit for practitioners in primary care on how to effectively find, engage and support people who are at risk of CVD and also experience inequalities and/or multiple disadvantage.

Impact
The impact of the toolkit for practitioners in primary care is yet to be assessed.
