Purpose
To improve health and well-being of the local population of Eccles and Irlam in Salford and to reduce pressures on the primary health care system by signposting people to more appropriate services.

Activity
The Health to Well-being Network was a one-year partnership with Salford Health Matters CIC, Inspiring Communities Together and Salford CVS. The Network aimed to reduce social isolation, reduce non-medical-related G.P. consultations, and to improve links between general practices and the wide network of community assets.
All general practices were given access to, and actively promoted the uptake of, the waytowellbeing.org portal. A team of Volunteer Wellbeing Champions helped people to navigate the portal and to access the community assets that they needed.
Unlimited Potential developed and delivered its model of well-being mentors as a single point of access for local support to help people engage with community assets more quickly and effectively.

Impact
The self-evaluation indicated evidence of positive impact of the well-being mentors on people, and a consequent reduction in demand on staff and services in general practices.
