Purpose
To help ‘clinically extremely vulnerable people’ maintain, and hopefully improve, their health and well-being in ways that suit them.

Activity
Through in-depth conversations, we asked a diversity of people about what they enjoyed doing and what they most valued in their lives. When we asked people how they managed their experience of having long-term health conditions, many people told us that they “take things day by day.”
We heard most frequently six themes: being respected; health; understanding yourself; purpose and being valued; quality relationships; and enjoyment.

Impact
We shared our learning in the Day By Day report with local authorities and the NHS to help inform the design and development, with people with long-term health conditions, of innovative approaches that support them to have the best possible quality of life, by addressing the six themes.
