Purpose

To optimise outcomes for young adults with renal conditions, by improving attendance and engagement with specialist renal care.

Activity

 

There was a relatively high non-attendance rate at the specialist Young Adult Renal Clinic at Salford Royal Hospital, meaning poor clinical outcomes for some people. 

With the support of the clinical team, we engaged and worked with a group of young adults who attended the clinic. We focussed on them as people (rather than as patients) and identified their strengths and skills. 

Some of the young people then did a five-day community reporter course with Inspiring Communities Together. With their new and enhanced skills, the team produced a video promote the young adult renal service at the hospital, supported by an experienced community reporter.

Impact

The wayfinding video was adopted by the NHS foundation trust. It led to improved attendance at the Young Adult Renal Clinic, enabling effective treatment for more people, and improved clinical outcomes.
