Salford is shaping a new relationship with local people to improve population health in the city. One part of this is the creation of an overarching social prescribing programme for Salford. The role of a Community Connector is critical to the delivery of social prescribing.

 

You will support individuals to access local groups and activities, adopting a strengths-based approach to improve their own health and well-being. You will with local communities to identify and develop opportunities to meet the residents in the area. You will also liaise with health and social care professionals to support the development of the programme.

 

With good engagement and communication skills, you are able to support local people from a wide range of backgrounds to make their own decisions. You can also develop and maintain relationships with professionals in public services. You are able to work independently and to be part of a team.

 

You have knowledge of the determinants of health and well-being, and are familiar with behaviour change and the barriers that people may face. You have experience of the benefits of lifestyle changes, and of monitoring and reporting outputs and outcomes of projects or services.
